Susan Foster

Director of Care Management  

Sue Foster is the Director of Care Management for Shields Health Solutions.  Working on-site at a Shields-partner health system, she manages teams of licensed nurses, social workers and care coordination experts for an Accountable Care Organization with 52,000 beneficiaries that includes 9 hospitals, 4 federally qualified health centers , 480 primary care physicians, and more than 1,300 specialists. In her role, Sue works closely with her Medical Directors and a team of analysts using predictive analytics to proactively find patients that can benefit most from care management, including home visits, telephone counsel, tele-health, and supportive attendance at patient physician visits.  Her primary goal is to make a positive health impact for all of the ACO beneficiaries, including those living with chronic illnesses, and heavily burdened by social determinants of health. In doing so, Sue is meeting the Triple Aim of health care: improved outcomes, lower costs and higher patient satisfaction. Prior to joining Shields, Sue served in the roles of Clinical Services Liaison and Nurse Liaison for two national home infusion companies, Nurse Recruiter for a large academic health center, Ambulatory Case Manager, Inpatient Case Manager and Assistant Nurse Manager for an inpatient telemetry unit.
